| OPTION

$3750 Entrance Fee
is paid upon admission. It is refundable (minus $250 processing)
if the resident leaves within the first three (3) months of residency.
A lower monthly fee is the benefit of paying the Entrance Fee. |

to pay an Entrance Fee, choose Option B. You will pay a higher
monthly fee. |

to pay an Entrance Fee now, choose option B for a trial period of
three (3) months. You reserve the right to choose Option A
($3750 Entrance Fee) within the first three (3) months of residency. |

Features and Services
- Three meals each day are served in a beautiful central dining room by Clark Jeary staff.
- A “Quick Response” call system is included in your bedroom and bathroom. It will be monitored by
Clark Jeary licensed nursing staff 24 hours a day.
- Medication assistance is available for an additional
$109 - $145 per month.
- Weekly
bath, dressing, grooming, personal care assistance, meal and activity
reminders, daily bedmaking, and removal of trash as needed.
- Respite
care is available.
- Weekly housekeeping is provided. This includes vacuuming carpets, cleaning of bath floors, dusting of horizontal surfaces and cleaning of toilet, shower, sinks and counters.
- Weekly laundry service includes pick-up and delivery of our towels and bed linens. Clark Jeary staff will take care of the linen changes. Coin-operated washers and dryers are available for your personal use.
- All utilities, except telephone and cable television, are included in your monthly rate. Maintenance of the heating and cooling systems, landscaping, snow removal, trash collection and pest control are included.
- All apartments include carpet, coordinated vinyl flooring in bathroom, window coverings, individually controlled heating and air conditioning, smoke alarms and a sprinkler system for fire protection.
- Second occupant rate is
$550 per month.
- Additional services available at
additional cost include transportation to medical appointments, scheduled shopping and outings, guest meals, meals or room trays for short-term
illnesses, beauty/barber shop services, an attached garage, telephone and cable television, and hi-speed internet service.
